certs and bla bla bla for form5s (CBNers only)
please make sure that you have photocopies of these:
*a copy of your PARENTS IC ( i know, this is WEIRD) and it's both mother and father.
*a copy of PMR result slip ( now, where's mine? ah~ still in the office~ haven't collect it. how GREAT. i'm gonna get serious scolding~)
*a copy of YOUR OWN IC ++ BIRTH CERT!!
*a copy of each your CERTS that you wish to be included. You can submit up to 50 certs. Which means 50 is the maximum.
put the photostated CERTS into an envelope and bring your original certs together with the above documents to be verify by your dearest class teacher. She will then sign it and you can have your original certs back. =)
